The first step is to contact us to set up an initial phone call with a psychologist. The best way to do that is to submit the secure form below. You also have the option to call or email us. This call will take between 10-15 minutes and we’ll talk about the problems you’re facing and whether our psychologists are the right therapist for you. This initial call is of no charge to you. On this call, we will focus on what is motivating you to seek treatment now as well as scheduling an initial session.
After this, we will set you up with a patient portal on Simple Practice and email you a link. The patient portal will give you access to documents for you to read and fill out, including office policies, informed consent, personal questionnaires, your payment information, and other items.
If you are using your insurance, we will ask you for a copy of the front and back of your insurance card so that we will be able to verify your insurance coverage.
